COVID-19 Related Report Form

University members must submit the COVID-19 Related Report Form when either they or a family member / co-inhabitant arrives in Hong Kong from abroad or the University member has been exposed to an individual or a close contact of an individual that is a confirmed COVID-19 case. The reporting requirement also applies to University members arriving from Mainland China, Taiwan or Macau, in case there is any relevant travel or contact history. Please also be aware that the travel and contact report covers the 14 days prior to the date of reporting, even if this covers a period before the effective date of the global travel restriction.

Any member of the University who has traveled from abroad, including the Mainland, Taiwan or Macau, must follow the Government’s quarantine arrangements and notify their department/ supervisor/ UG coordinator/ program director immediately and take necessary follow up actions as requested by the University.  Violating these guidelines may result in disciplinary action.

University members who are asked by a medical doctor or required by the Government to take a COVID test or who have a household member undergoing COVID-19 test should submit a Report Form.
